Does the same thing happen if the delay is in the post fader position?

Yes, but here's something interesting: if I have the Echo/Delay instantiated in both the pre and post positions, set them the same with both unmuted, the echo works perfectly with the audio file at 00:00:00. If I mute EITHER one, I get only a single echo.

Can you try removing all automation, leaving just the echo in the post position with more than one region on the track? No mutes.

OK. It appears the echo plugin needs a pre-roll equal to the delay length to work properly. The reason it appeared to be working for me was I had a longer sample than the delay setting.
So, I can confirm this now. The obvious fix is to configure the session for some pre-roll. You can still have an accurate time count by having the timeline start at 23:58:00:00 and still having the session start @ 00:00:00:00.
